The following are standard forms to be used in conjunction with photocopies to show that they have been made under this guide. These forms may be photocopied, or downloaded from http://www.apra-amcos.com.au/musicconsumers/musicatevents/eisteddfods.aspx Form 1 - Performer's declaration Form for Eisteddfods and Music Competitions To be attached to each entry form. Teachers may sign one form on behalf of several students on the one entry form. Form 2 - Temporary Copy for the Use by an Adjudicator in an Eisteddfod/Music Competition To be presented at the Registration Table attached to each piece of photocopied music for the adjudicator. Form 5 - Temporary Copy for the Use by an Accompanist in an Eisteddfod/Music Competition This form is to be attached to the accompanist's copy of the music. |
